This is a notice that one or more of your reports is missing. Please review
your records to see if the report(s) were submitted.

| Receive a notice of an investigation of an
unemployment claim?

Someone has filed for benefits claiming that they worked for you or your
business. The auditor listed on the notice needs to verify that person's
employment with your company. Please have available the records that are being
requested. If you feel the person was not an employee of your business, but in
business independently, have documentation available. If you have questions,
please call the local compliance office listed
on the notice.

| Disagree with an audit finding?
|

You have the right to appeal any decision
within 30 days. When the audit is completed, ask the auditor to have a
determination letter mailed to you. You must make your request
for reconsideration in writing, citing the reasons why you disagree.
Remember to include your employer account number on any correspondence.

| Fraudulent employer?
|
 |
|
If you know of any employer who may not be properly reporting employees, write
to ODJFS, Compliance Section, PO Box 182404, Columbus, Ohio 43218-2404. Please
provide as much detail about that employer as you know. For example, names and
numbers of workers, work sites, type of work, hours worked, how often paid, how
much paid, etc. All information will be kept confidential. Please provide your
name and phone number in case we need to verify some detail. Your name will not
be shared with the employer.

| Where can you get extra forms?
|

|
If you need contribution or wage record reporting forms, any local
Compliance office will provide them at no cost. For those employers
with more than 20 employees, supplemental wage record reporting forms may also
be obtained from local Compliance offices, ordered from ODJFS, Contribution
Section, PO Box 182404, Columbus, Ohio 43218-2404 or by calling (614) 466-2319.
