To be enrolled to provide waiver services as an RN, LPN, Independent Daily Living Aide, or Independent Daily Living Non-Aide, or to enroll to perform services with the Ohio Home Care waiver program such as Home Modifications, Supplemental Transportation, Out of Home Respite, please contact the Bureau of Community Services Policy at 614-466-6742.
