Transition Coordinators & Case Managers

Welcome to the HOME Choice Web page for Transition Coordinators and Case Managers, two important components in the success of the HOME Choice program. To the right, you will find a tool box, with most of the information you will need to get started in one of these roles.

What is a HOME Choice Transition Coordinator?
Transition Coordinators are unique to the HOME Choice program. They facilitate the steps needed to help a consumer transition from a long-term care facility to a home in the community. Such steps include helping the consumer locate housing, purchase materials and supplies for living and becoming integrated into the community, and working closely with the consumer's case manager.

All HOME Choice participants will choose a Transition Coordinator from among the following entities:

  • Brain Injury Association of Ohio Network
  • Centers for Independent Living
  • County Boards of MR/DD
  • Long-Term Care Ombudsmen
  • Ohio Department of Mental Health-designated mental health providers.

The Transition Coordinator roles ends when the consumer leaves the long-term care facility and moves into the community.

What is a HOME Choice Case Manager?
The HOME Choice case manager provides traditional case management services, as well as works closely with the Transition Coordinator. Case management is a collaborative process of assessing, planning, facilitating and advocating for options and services to meet the consumer's health needs. All HOME Choice participants will be assigned a case manager through the agency of which their Waiver or State Medicaid Plan Services is administered.

In the MR/DD system, the Services and Support Administrator can perform both transition coordination and case management services. In all other systems, the Transition Coordinator and Case Manager are separate functions.
