These links provide access to useful documents:

  1. A template with the basic outline of the program standards, or plan, that must be submitted to register an apprenticeship program in Ohio,
  2. A Program Summary outline that is submitted when regsitering or updating a program.
  3. A guide to the Equal Opportunity provisions required in the program standards,
  4. credit voucher to be used for validating credit for an apprentice,
  5. The Apprenticeship Agreement form.
  6. The Apprenticeship Cancellation form, and
  7. The Application for an Apprentice Completion Certificate.

Prospective program sponsors can download the standards template and add the appropriate details for submittal to the nearest apprenticeship service provider (click for contact list).  The representative will review the document and forward it to the Ohio State Apprenticeship Council (OSAC).  OSAC approval of the standards constitutes registration of a program.  Existing program sponsors can also use the template as a format for program revisions, and mail directly to the central office for review.

As of October 10, 2011, OSAC has migrated to electronic apprentice transactions that sponsors must conduct online.  Please contact the central office to obtain the username and password for individual sponsor accounts (see contact list below).  Sponsors can download the Apprenticeship Agreement form, credit voucher, Apprenticeship Cancellation form, and Certificate Application, and fill them out for submittal to the central office until individual accounts are created.  These forms are available in a PDF and MS Word format, and in each case can be completed in two ways:

  • They can be filled out electronically on any computer with Word and/or Acrobat writing software, and then mailed to central office (see address below), or e-mailed to central office contacts (see contact list below).
  • Sponsors without this software can print the blank forms and fill them out by hand.

To submit a completed document for processing by the OSAC, please mail to the central office, or e-mail it to one of the following individuals listed below:
