PASSPORT Waiver Program
The PASSPORT Waiver program benefit package consists of adult day health programs, environmental accessibility adaptations, home-delivered meals, personal emergency response systems, specialized medical equipment and supplies, chore assistance, independent living assistance, nutritional consultation, personal care services, social work and counseling, and transportation.
Three local Area Agencies on Aging act as regional administrators and provide case management services.
Who is eligible?
Consumers eligible for the PASSPORT Waiver program must be eligible for Medicaid, need at least an intermediate level of care and be age 60 or older.
